The Importance of Molecular Monitoring in the Treatment of AML

In an interview with Targeted Oncology, Eytan M. Stein, MD, reviews the current treatment landscape of acute myeloid leukemia, underscores the importance of molecular monitoring, and highlights where the field is headed.

Now that multiple treatment options exist for acute myeloid leukemia, the addition of molecular monitoring is needed to help determine how to best sequence these agents, said Eytan M. Stein, MD.

In the newly diagnosed setting, the FLT3 inhibitor midostaurin (Rydapt) is being used in combination with induction chemotherapy in patients who are well enough to undergo chemotherapy. For those who are not fit enough, physicians are combining hypomethylating agents (HMAs) or low-dose cytarabine with venetoclax (Venclexta).

In November 2018, the FDA granted an accelerated approval to venetoclax for use in combination with azacitidine or decitabine or low-dose cytarabine in the treatment of newly diagnosed patients with AML who are aged 75 years or older or have comorbidities that make them unfit for induction chemotherapy. The approval was based on positive data from 2 early phase trials: M14-358 and M14-387.

In the M14-358 trial, combining venetoclax with azacitidine was found to result in a complete remission (CR) rate of 37% and a CR with partial hematological recovery (CRh) rate of 24%. The CR rate with the combination of venetoclax and decitabine was 54%, whereas the CRh rate was 7.7% with the combination. Further, results from the M14-387 trial showed CR and CRh rates of 21% in patients who received venetoclax with low-dose cytarabine.

“That’s really a paradigm shift in how we treat AML both in the fit and unfit populations,” said Stein, who is an assistant attending physician on the Leukemia Service at Memorial Sloan Kettering Cancer Center in New York, New York.

Several new agents have also received FDA approval for use in the relapsed/refractory setting, including the IDH1 inhibitor ivosidenib (Tibsovo), the IDH2 inhibitor enasidenib (Idhifa) for patients withIDH1/2-mutated disease, and the FLT3 inhibitor gilteritinib (Xospata) for those withFLT3-mutated disease. Stein is hopeful that oncologists will soon be able to add another FLT3 inhibitor, quizartinib, to their treatment armamentarium.

“It is crucially important that when physicians see a patient with newly diagnosed AML, they check for a variety of molecular genetic abnormalities at the time of diagnosis,” Stein said. “It is just as important [to check for these abnormalities] at the time of relapse, as well, because they can change from the time of diagnosis to the time of relapse. Doing so will help you come up with the best treatment choice for your patient.”

TARGETED ONCOLOGY:What are some recent advances that have been made in the treatment of patients with AML?

Stein: Our treatment options, up until very recently, were really quite limited. Patients could either receive induction chemotherapy with daunorubicin and cytarabine or get HMAs—that’s in the newly diagnosed setting. What has happened over the past few years is that there has been an explosion of new treatments for [patients with] AML.

[Specifically], in the newly diagnosed setting, we now combine the FLT3 inhibitor midostaurin with induction chemotherapy; that’s obviously for patients who are well enough to receive induction chemotherapy. For patients who are not well enough to get induction chemotherapy, we’re now combining HMAs or low-dose cytarabine with the BCL-2 inhibitor venetoclax.

There is also the combination of a drug called glasdegib (Daurismo), which is a hedgehog inhibitor, with low-dose cytarabine. In addition, the liposomal formulation of daunorubicin and cytarabine, called CPX-351, was FDA approved for patients with therapy-related AML and AML with myelodysplasia-related changes. As opposed to 2 or 3 years ago, when our treatment options were limited, it is now actually quite complicated to treat patients with newly diagnosed AML [because there are so many options to navigate]. 

In the relapsed/refractory population, we now have 3 new drug approvals [from the FDA]; there may be 4 soon. One is the IDH1 inhibitor ivosidenib, 1 is the IDH2 inhibitor enasidenib for [patients with]IDH1/2-mutant AML, and 1 is the FLT3 inhibitor gilteritinib for those withFLT3-mutated AML, and we expect—hopefully, in the next 3 or 4 months—that another FLT3 inhibitor named quizartinib will also receive approval.

TARGETED ONCOLOGY: What is the importance of molecular monitoring in AML?

Stein: You really need to check the molecular genetics of newly diagnosed patients with AML because [that will provide you with] an idea of a patient’s prognosis. We’ve now been able to integrate not only single molecular genetic abnormalities but also determined how these molecular genetic abnormalities and their co-occurrence can influence prognosis, and that becomes important when you’re thinking about a patient who might be eligible for a bone marrow transplant, whether they need a bone marrow transplant at the time of their first remission. That’s the first thing.

The second thing is how we use this molecular monitoring to inform how patients are doing while they’re on their treatment journey. After they get their initial induction chemotherapy, we can check and see whether there’s something called minimal residual disease [MRD] that is left over, and if there is MRD left over, that gives us an indication of whether a patient might relapse or might not.

Finally, checking molecular genetic abnormalities has really affected how we treat our patients, because in the newly diagnosed setting, we have FLT3 inhibitors that get combined with chemotherapy. In some settings, you might use single-agent IDH1 or IDH2 inhibitors. In the relapsed/refractory setting, we now have FLT3 inhibitors and IDH1/2 inhibitors that will be used for these patients.

TARGETED ONCOLOGY: What ongoing research in the field are you excited about?

Stein: With these new drugs that have been approved, there is now an explosion of research [into 2 areas]. One is [figuring out] how to combine these drugs to improve outcomes for the patients, and [the other is deciding] the best setting. That is, is it better to use these drugs in the newly diagnosed setting? Is it better to use these drugs in the relapsed/refractory setting? Which drug do you use first? How do you sequence the drugs? There are tons of questions that we will spend the next 5 to 10 years answering.

TARGETED ONCOLOGY: Do you have a take-home message for your colleagues who are treating patients with AML?

Stein: The main takeaway is that they should understand which molecular genetic test they need to order. They should also understand which drugs can be given if they get positive results on certain molecular genetic tests. They should feel comfortable ordering these tests and also reaching out to experts in the field to help them along the way. Many community oncologists don’t see so much AML [in their practice], so [when they do,] it’s important that they reach out to the folks who have used these drugs a fair amount.

TARGETED ONCOLOGY: Where do you see the field heading in the next decade?

Stein: [Despite this progress, it’s] still AML, and most of these patients, unfortunately, still succumb to their disease. In the next 10 years, we’re going to be seeing dramatic improvements in the overall survival of these patients and improvements in the quality of life. Hopefully, in what was really a disease that had few treatment options and was in some ways as bad as something like pancreatic cancer, we’re going to see it, hopefully, become a chronic disease, sort of like chronic leukemias.
